| Submitting Your Sample For a DNA Test |

Step 1
Print and fill out the order form.
Step 2
Take 2 Q-Tips and cut off one end on each. Holding the first Q-Tip by the cut end,
swab the inside of your left cheek with the cotton end, for about 30 seconds. Take
the second Q-Tip and holding it by the cut end, swab the inside of your right cheek,
for about 30 seconds. You should use firm, but not extreme pressure.
Step 3
Take a paper envelope and label it as "Cheek Swabs" . Place both Q-Tips into the
paper envelope and seal it. DO NOT place the swabs into a plastic bag, the swabs
need to be able to dry, otherwise the moisture will degrade the DNA.
Step 4
If the stained sample you are submitting for a DNA test is dry, enclose it in a ziplock
bag (it is recommended that you wear gloves while handling the sample to prevent
DNA contamination). If the stained sample is moist, enclose it in a paper bag.
Label the bag as "FEMALE DNA Test".
Step 5
Send the order form, payment (credit card, money order, cashiers check) and all
samples to:
